A normal Space Marine can carry a storm shield and a Thunderhammer at the same time right?

Carry- yes. Use- no.
Thunderhammer is too heavy to use it singlehandedly without Terminator armour.
But, judging from some works, you can use power weapon with a stormshield while in normal Tactical Marine armour.

What about an Astartes that has been heavily augmented with cybernetics and biological/chemical upgrades?

Generally considering, you're not going to get the optimal performance out of the thunder hammer/ storm shield combo unless you're in Terminator Armor because of the weight and unwieldiness of the hammer (thus needing the added strength of Terminator Armor) and the destructive potential of the weapon combo.  But all-points-considered you can always find at least one story example of a special-snowflake or unique-exception to the rule.  However, the wielder would probably be suicidaly-crazy and ridiculously-clumsy with the weapon-combo allowing any competent space marine with a power sword to dodge around the unwieldy weapon and skewer the nut-job.

Also Astartes already are biologically and chemically upgraded, and as stated in "Wrath of Iron", Iron Hands cybernetic implants are only at best on par with Astartes biology, and they have the best there is.
